What is BHRT?

BHRT uses bioidentical hormones with the same molecular structure as the ones your body naturally produces. Bioidentical hormone replacement therapy is formulated to closely align with your body’s natural rhythms. Bioidentical hormones are often custom-compounded to match your individual dosing needs.

Already Have a Prescription?

If your provider is already experienced in BHRT and sends us a compounded prescription, we can begin preparing your medication immediately. A formal pharmacist consultation is not required if your provider is comfortable managing your hormone therapy independently.

Your provider remains your prescriber and directs your medical care. Pharmacists do not prescribe medications. Our role is to provide clinical insight, education, and monitoring to reduce the burden on your provider while improving your experience and results. This is true collaborative care — your provider, your pharmacist, and you working together.

Patient Instructions & Expectations

Before Your First Appointment

  • Complete the Confidential Female Hormone Evaluation Form (link to this form will be sent in your appointment confirmation email after you schedule an appointment with us through the schedule a consultation button).
  • Ensure you’re up to date on relevant screening exams (Pap, Mammogram, Well Woman, etc). We’ll help review current guidelines with you.

Symptom Journal

Track any changes or side effects once BHRT begins.

Lab Work

To monitor hormone balance:

  • We MAY or MAY NOT request serum (blood) or saliva baseline labs before starting therapy.
  • Once you start on a compounded BHRT prescription, we often require labs 3 months after starting, then every 6–12 months thereafter. These follow-up labs will be required to be saliva labs instead of serum if your BHRT RX is a topical cream. Saliva kits are sold at The Medicine Store and can be taken home for collection.
  • Baseline labs you could ask your provider to order: Estradiol, Estrone, Progesterone, Free & Total Testosterone, DHEA, Vitamin D, & morning cortisol. (however, we will help coordinate which labs should be ordered during your appointment. So, it is not necessary to do this before meeting with us).

Important:

  • Serum labs must be drawn exactly 5–6 hours after your dose.
  • Saliva labs, you must follow the kit’s exact timing guidelines.
  • Never stop BHRT doses before testing unless told otherwise.
  • Forward all lab results to our pharmacy for review.

Topical vs. Oral BHRT | Serum vs. Saliva Labs

  • We often recommend non-oral forms (like creams or troches) to avoid first-pass metabolism through the liver and mimic natural hormone delivery.
  • For topical hormones, saliva testing gives more accurate insight than serum.
  • Serum labs are still valuable—especially for oral forms like troches.
  • We use ZRT Laboratories for saliva testing kits (available in-pharmacy).

Do I have to pay for a consultation to get my BHRT prescription filled?

No. If your provider is already comfortable managing your BHRT and sends a prescription directly to The Medicine Store, we will fill it like any other prescription. The $200 consultation is only for those who want the added benefit of our pharmacist-led collaborative monitoring and program.

What’s the difference between BHRT and regular HRT?

BHRT uses hormones identical to your body’s own. Traditional HRT often uses synthetics with altered structures.

How long does it take to see results?

Some see improvements in weeks. Full hormone balance may take up to 6 months.

Can my provider work with your pharmacist?

Yes. We send recommendations, your provider signs off, and we stay in sync through the entire process.
