To schedule an appointment at The Medicine Store, Basehor, first choose a service from the drop-down menu and then click “next.” There will be several different appointments types to choose from. All vaccine appointment slots will only be for 5 minutes, but you will be required to stay in the pharmacy for 15 minutes after your appointment time to be monitored by our staff. The BHRT consult appointment slot will be for 1 hour. If you are scheduling for a COVID-19 vaccine, be sure you have read all of the information we have below on our homepage, and also be sure you are choosing the type of COVID-19 vaccine (Moderna, Pfizer-BioNTech, or Janssen/Johnson & Johnson) you want to receive from the service drop-down menu. There is a pediatric version of the Pfizer vaccine for those ages 5-11 and a separate version for those who are ages 12 and over, so if you want Pfizer as your COVID-19 vaccine, be sure to choose the age-appropriate vaccine when scheduling an appointment. If you are scheduling an appointment for a COVID-19 vaccine booster, choose the type of vaccine you would like to receive as your booster from the service drop-down menu and then one of the questions will ask you if it is your 1st initial dose, 2nd initial dose, 1st booster dose or 2nd booster dose. It will have an option for an “additional dose,” but the “additional dose” is only for those who are moderately to severely immunocompromised. You can click on the “EUA fact sheet” links below to see whether or not you are eligible for a first or second booster or an “additional dose.”
Appointments for both rapid antigen and PCR COVID-19 testing are also available below on the service drop down menu. Only 1 appointment per vehicle is necessary. So, if more than one person is coming with you to be tested, schedule the appointment for just one of the people that will be in the vehicle and one of the prompts during the scheduling process will ask you how many people will be with you to be tested in your vehicle. We only open appointments for this service a few days in advance, since we are not always sure how many tests we will have in stock from the state. Also, be sure to schedule for the correct type of COVID-19 test you want to receive (PCR or rapid antigen test). We are a community testing partner with the state of Kansas. This means the State is funding this program, and, therefore, we are able to offer these tests at no out-of-pocket cost to the public. The State is currently funding both types of tests.
Once you have chosen a service from the drop-down menu, you will then choose a date and time for your appointment and continue following the prompts until you get a confirmation. The confirmation is easy to miss. It will show up under the service drop-down back here on the homepage. You should also receive a confirmation email (check your inbox and junk mail). If you do not receive the confirmation email, please give us a call and we can confirm the appointment over the phone for you. Thank you and we look forward to meeting with you!

There will be NO OUT-OF-POCKET COST to ANY patient for the COVID-19 vaccine regardless of insurance coverage. However, as a provider we can bill your insurance (if you have it) for the administration of the vaccine, so please bring your medical and pharmacy insurance cards to your vaccine appointment. The Flulaval Quadrivalent Flu vaccine will be subject to the copay set by your insurance with a max out of pocket cost of $40 if you do not have insurance.
If you are on Medicare please bring your red, white & blue Medicare card.
Last, please bring a photo ID or at least some sort of identification so that we can verify the age of the patient receiving the vaccine.
Once you’ve scheduled an appointment, there is an immunization screening and consent form that needs to be filled out. Click the link below to print off the form. There is a one form for the COVID-19 vaccine and a second form for all other vaccines. If you are receiving the COVID-19 vaccine and another vaccine at the same appointment, you will have to fill out both forms. If you don’t have the ability to print the form(s) out at home, we will provide the paperwork for you and you can fill it out at the pharmacy during your appointment.
If you are unable to schedule an appointment with us, we recommend checking with your local health department for vaccine availability. The Leavenworth County Health Department has been hosting several walk-in vaccination clinics. Please refer to the health department website for details regarding these clinics:
